Payment Policy
Payment is due when services are rendered.
Consultations may have a $50 office charge that is not covered by your insurance. If you decide to proceed with care at that appointment, the $50 will be credited to the services provided.
We accept most private insurance, Medicare, and worker’s compensation insurance.
Payment options – We accept Visa, Master Card, Discover Card, Cash, and Checks.
Cancelled and Missed Appointment Charges
If you cancel or reschedule your appointment more than 24 hours in advance, there is no charge.
After the second time you have a same day cancellation or missed your appointment, there will be a $50 charge for the missed appointment. This charge is not covered by your insurance.
No Surprises Act
Due to the nature of Prosthetics and Orthotics, there often is no single cost line item for a custom prosthesis or orthosis due to Centers for Medicare & Medicaid Services’ HCPCs coding system.
BEFORE we provide any care, we are happy to provide a written itemized estimate for services and will explain your anticipated financial responsibility based on the information provided by your insurance company. Insurance coverage is never a guarantee, but if we are in-network with your insurance, we will only bill you the amount your insurance company lists as ‘patient responsibility’ for covered service.
For services that are NOT covered by your insurance company, or if you are self-pay, we will bill at the prevailing Medicare Allowable rate for each of the appropriate HCPC codes.

Refund Policy
We provide professional services, our time spent providing the evaluation, treatment, and follow-up care. Often, the professional service is accompanied by a physical device or item.
If the device/item matches what is listed on your prescription, but is unused, unaltered, not damaged, and in the original packaging, a return within 14 days will be subject to the $50 office consultation fee plus any external costs we incurred in providing your care, which is not covered by insurance.
We cannot accept returns or issue refunds for items that were custom fabricated, special ordered, modified, altered, used, worn, not in the original packaging, or beyond 14 days.
